COMMON GRANT APPLICATION
The Foundation will accept the Colorado Common Grant Application.
Additional information may be requested at the time of the application.
INFORMATION FOR PREPARATION OF PROPOSALS
If the Common Grant Application is not used, the following information
is required.
COVER LETTER. One page which summarizes the request, the
amount requested, and how the request relates to the funding interests
of the Foundation. This letter must be signed by the Board President
and the CEO of the requesting organization.
NARRATIVE. (Not to exceed 2-4 pages)
1. Organization Summary
a. Organization name and address
b. Chief Executive Officer
c. Contact Person (if other than CEO)
d. Office phone number, fax number, and web site
e. Mission statement and brief statement of goals
f. Brief historical summary
2. Description of the Request
a. Amount requested
b. Brief statement of the issues to be addressed; description
of constituency to be served (include number to be served);
and how they will benefit
c. Description of the goals and objectives for the proposed
grant and activities planned to accomplish these goals
d. Timetable for implementation
e. Expected results and plan for measuring success of the project/program
f. Distinctive characteristics of this project; explain any
duplication with similar agencies
g. Participation of volunteers
h. Relationship, if any, to previous grants awarded to the organization
by the Bonfils-Stanton Foundation
ATTACHMENTS:
Complete copy of most recent 501(c)(3) Internal Revenue
Service designation
List of Board Members, Officers and Directors of the organization
with occupations and/or community affiliations noted
Current organizational budget
Proposed program/project budget
Year end financial statements (audited, if available)
Most recent financial statements (a balance sheet and an
income/expense statement)
List of major contributors for the last year, including
amounts
Names of other organizations to whom you are applying for
the same program or project
ADDITIONAL GUIDELINES
Only one copy of the proposal is requested.
Please do not staple the proposal or use binders or notebooks.
Organizations may submit only one proposal in a calendar
year.
Unless invited by the Foundation, organizations whose proposal
has been declined may not reapply for a period of two years, and
then only if the request has been materially changed.
